Analgesics

Fentanyl is a very short acting, potent analgesic. Often used in critically ill animals because it allows close control of sedation and analgesia due to its short half life. This dose can be used before starting a Fentanyl CRI, but will not provide long acting analgesia. This dose can also be given SQ or IM, but the circumstances where you would use fentanyl this way would be unusual

Hydromorphone (IV/IM)

Hydromorphone is a long acting analgesic that is good for pain. In the author's experience, givingly slowly IV is associated with less emesis than giving IM. Commonly used as a portion of a premed for short procedures requiring anesthesia. Use hydromorphone cautiously in feline patients as it has been associated with hyperthermia in feline patients. Common side effects in canines include excessive panting and ptyalism.



Antibiotics
Amoxicillin/Clavulanic acid (Clavamox) (PO only)
13 - 25 mg/kg
Standard dose range
Amoxicillin/Clavulanic acid (Clavamox or human Augmentin) is a commonly used broad spectrum antibiotic. Give the lowest dose possible in this dose range based on tablet size. When using liquid aim for 15 mg/kg as some a small amount is often lost in administration. Dose q 12 h. Closest injectable is Unasyn [Ampicillin/sulbactam].

Unasyn (IV/IM)
22 mg/kg

Ampicillin/sulbactam (Unasyn) is a broad spectrum injectable antibiotic. It is essentially injectable Amoxicillin/Clavulanic acid (Clavamox). Dose q 8 h. This dose is rounded up to the nearest 10 in mg then to the nearest 0.1 ml and the milligrams are recalculated.



Antiemetics
Maropitant (Cerenia) (SQ)
0.5 mg/kg

Canine dose
1.0 mg/kg oral dose:
Cerenia should be used for no more than 5 consecutive days. Oral doses of 8 mg/kg can be used for up to two consecutive days for the prevention of motion sickness.

Ondansetron (Zofran) (IV, PO)

Ondansetron is an inexpensive antiemetic that can be dosed as an injectable q6-12 hours. Generally start with q12h and increase as needed. Higher doses can be used with chemotherapy.



Antifungals
Fluconazole (IV, PO)

Fluconazole is a front line of antifungal therapy because of its low cost compared with other options. Dose q 12 h.

Itraconazole (Sporanox) (PO only)
2.5 mg/kg
Standard dose
Itraconazole is more effective when the brand name is given although it is very expensive. Generally it is least expensive from online Canadian pharmcies. Dose q 12 h. Can double the dose and give q 24 h.



NSAIDS
Meloxicam (Metacam) (IV/IM)
0.1 mg/kg
Standard dose
0.2 mg/kg
Loading dose
Metacam is an NSAID that is convenient to dose because it comes in a liquid form. A single loading dose at 0.2 mg/kg can be given before starting at the maintenance dose of 0.1 mg/kg.

Carprofen (Rimadyl) (PO or SQ injectable)
2.2 mg/kg
Standard dose
Rimadyl is an NSAID that is relatively inexpensive. Only available as an oral medication or SQ injection. Can be given q 12 h as the listed dose or q 24 h at twice this dose (4.4 mg/kg). Causes less GI upset when given q12h.
